Nurse Practitioner/Physician Assistant - Internal Medicine-Weekends
About The Position
Evolutionary Healthcare is seeking compassionate and motivated Nurse Practitioners (NPs) and Physician Assistants (PAs) to join our growing team providing primary care services throughout post-acute and skilled nursing facilities in Central Florida. Requirements
- Current Nurse Practitioner or Physician Assistant license in Florida
- National Board Certification
- DEA license (or ability to obtain)
- Excellent clinical judgment and communication skills
- Commitment to compassionate, patient-centered care
Nice To Haves
- Experience in Internal Medicine, Family Medicine, Geriatrics, Hospital Medicine, Emergency Medicine, or Post-Acute Care preferred
- Skilled Nursing Facility experience is a plus, but not required
Responsibilities
- Provide comprehensive primary care services in skilled nursing and post-acute care facilities.
- Evaluate, diagnose, and manage acute and chronic medical conditions.
- Develop individualized treatment plans that improve function, mobility, and quality of life.
- Perform routine follow-up visits, admissions, discharge planning, and transitional care.
- Collaborate closely with nursing staff, therapists, specialists, and facility leadership.
- Educate patients and families regarding disease management and preventive care.
- Maintain accurate and timely documentation using electronic health records.
