About The Position

As an Advanced Practice Provider, you'll play an integral role in our inpatient and ambulatory care teams by providing high-quality, cost-effective care to patients. We are seeking a Nurse Practitioner or Physician Assistant to work in an innovative Home-Based Primary Care program working with older adults that are homebound. You will collaborate with a focused interdisciplinary team to care for patients. This role involves conducting comprehensive or episodic health history and physical assessment of patients/families with acute, chronic and/or potential health problems in the patient’s home. You will also provide appropriate direction to program and clinic staff regarding patient care issues.

Requirements

• If Nurse Practitioner: Master’s or Doctorate’s degree from accredited Nursing Program.
• If Physician Assistant: Bachelor’s or Master’s degree from accredited PA Program
• If Nurse Practitioner: Active RN and APRN certifications and licenses, including prescriptive authority, in state(s) of practice AND valid WI Drivers license and reliable vehicle
• Active board certification as a NP in area of clinical practice. These may include the following: Adult;; Adult/Gerontology Primary Care; Family; Gerontology;
• If Physician Assistant: Active PA license, including prescriptive authority, in state(s) of practice
• PA board certification
• Active DEA license or ability to apply for such license prior to or upon hire
• Active license or eligibility to obtain active licensure in state of practice. This may include licensing in Wisconsin and other state(s) of practice.

Responsibilities
