About The Position

The Nurse Practitioner or Physician Assistant will deliver comprehensive primary care, chronic disease management, and preventive health services to a diverse patient population via in-home visits (house calls) in a designated territory in the New York metro area. The exact geographic boundaries for the position will be mutually determined during the interview process. Focusing on geriatric and chronic care populations, the overall duties of the role include, but are not limited to: in-home assessments, physical exams, medical histories, and diagnostic evaluations; health screenings and wellness checks; preventive care, health education, and counseling; develop and implement individualized care plans; work in collaboration with patients, families, and interdisciplinary healthcare teams; manage chronic conditions (diabetes, hypertension, heart disease, etc.); order, interpret, and follow up on diagnostic tests, labs, and imaging; prescribe medications and treatments in accordance with scope of practice; administer immunizations; address urgent medical needs in the home when possible; provide timely follow-up care for patients discharged from hospitals, EDs, or SNFs; regularly evaluate patients to prevent readmissions; ensure continuity of care with primary care providers, specialists, and community resources; participate in quality improvement initiatives; provide culturally-sensitive and linguistically-appropriate care at all times. The practice uses the MDLand EHR. The provider will work with a majority Chinese-speaking patient population and must be fluent in Mandarin and/or Cantonese.

Requirements

  • New York NP or PA license
  • DEA
  • Relevant board certification (PA-C, FNP, AGNP, etc.)
  • 1+ year advanced practice experience in a relevant setting
  • Must speak Mandarin or Cantonese fluently

  • Experience with home or community-based care strongly preferred
