Full-Time Nurse Practitioner / Physician Assistant - Geriatric Care
About The Position
The Nurse Practitioner / Physician Assistant is responsible for delivering comprehensive primary care to a dedicated panel of geriatric patients. This includes visiting private homes, assisted and independent living facilities, and other group settings. The new provider will see medically complex, frail, geriatric adults who may have high clinical complexity, multi-morbidity, functional decline, mobility limitations, polypharmacy, cognitive impairment, caregiver burden, increased risk for avoidable utilization, and evolving goals of care. This requires providers to build and maintain therapeutic relationships with patients and caregivers, including providing emotional support and education as needed. The practice uses the AthenaCare EHR.
Requirements
- North Carolina NP or PA license
- DEA
- 1+ year advanced practice experience
Nice To Haves
- Previous home health or long-term care facility experience preferred
