About The Position

Humana's Primary Care Organization is a fast-growing provider of senior-focused, value-based care, with 340+ centers across 15 states under the CenterWell and Conviva brands. We're looking for a full-time Paneled Primary Care Nurse Practitioner who thrives in a collaborative, patient-centered environment and is passionate about managing a dedicated panel of adult and geriatric patients to drive quality outcomes and continuity of care.

Requirements

Graduate of an accredited Nurse Practitioner program (MSN or DNP)
Current national certification (AANP or ANCC)
Active, unrestricted NP license in South Carolina
DEA license and Medicare/Medicaid credentials (or ability to obtain upon hire)

Nice To Haves

2+ years of Nurse Practitioner experience
Experience managing a patient panel in a primary care or value-based care setting
Background in Medicare Advantage or geriatric-focused care
Experience with Athena EMR
Familiarity with HEDIS quality metrics, STAR ratings, and Medicare guidelines

Responsibilities

Manage and provide primary care services to a designated panel of senior patients in a value-based care model
Drive panel performance through quality metrics (e.g., HEDIS, STARs, risk adjustment) and patient engagement initiatives
Coordinate care across interdisciplinary teams, including specialists, behavioral health, and care management
Report to the Associate Medical Director
Work fully onsite in Homeland Park, SC
