About The Position

Expanding practice seeks an Inpatient Palliative Care NP to work in Findlay & Lima, Ohio! Join a collaborative, mission-driven Palliative Care team, working alongside experienced NPs, a Hospice Medical Director, and a Palliative Care Medical Director in a supportive, interdisciplinary environment. This is a full-time inpatient position, with 75% of the time spent at a hospital in Findlay, OH, and 25% at a second hospital in Lima, OH. The role involves a Monday-Friday schedule, with shared evening and weekend telephone call rotation. Providers follow patients throughout their hospital journey, focusing on symptom management, care coordination, and transitions of care. The position also includes providing inpatient rounding for General Inpatient (GIP) hospice patients at the second hospital as needed. New graduate NPs with a passion for palliative care are encouraged to apply.

Requirements

Certified Nurse Practitioner (Acute Care NP preferred; palliative care or hospice experience welcomed)
New graduate NPs encouraged to apply
Strong communication, interpersonal, and organizational skills
Comfortable navigating complex conversations related to serious illness, prognosis, and end-of-life planning
Ability to work independently while collaborating within an interdisciplinary team

Responsibilities

Perform inpatient palliative care consultations and follow-up visits
Manage symptoms for patients with serious, chronic, and life-limiting illnesses
Facilitate goals-of-care discussions and advance care planning with patients and families
Assist with discharge planning, including transitions to home-based palliative care or hospice
Provide inpatient rounding for hospice General Inpatient (GIP) patients at the second hospital as needed
