About The Position

This role involves providing palliative care to patients in a hospital environment, serving a diverse patient population. The nurse practitioner will educate caregivers on patient care, including disease education, advance care planning, medication administration, and end-of-life expectations. The position requires coordination of care with community medical providers and other disciplines, and participation in an Interdisciplinary Team (IDT) which meets daily. Collaboration with inpatient hospice clinical liaisons is also a key aspect of the role.

Requirements

• Must be comfortable educating caregivers on patient care.
• Must be comfortable educating caregivers on disease education, advance care planning, medication administration, and end-of-life expectations.
• Must be able to coordinate care between patient's community medical providers and other disciplines.
• Must be able to provide updates, obtain and place new orders.
• Must be able to collaborate with inpatient hospice clinical liaisons.
• Must be able to participate in an Interdisciplinary Team (IDT).

Responsibilities

• Provide quality care to patients in a hospital environment, regardless of socioeconomic and cultural backgrounds.
• Educate caregivers on patient care, including disease education, advance care planning, medication administration, and end-of-life expectations.
• Coordinate care between patient's community medical providers and other disciplines, providing updates, and obtaining/placing new orders.
• Participate in daily Interdisciplinary Team (IDT) meetings to review patient assignments and discuss complex cases.
• Collaborate with inpatient hospice clinical liaisons to coordinate and facilitate patient care at end-of-life.
• Conduct consultation visits (up to 90 minutes) and follow-up visits (15-45 minutes).
