About The Position

Signify Health, part of CVS Health®, is seeking a Full-time Clinician (Nurse Practitioner or Physician Assistant) to provide In-Home Health Evaluations. This role involves engaging with people in their homes to help bridge gaps in care and build trusted relationships to improve health outcomes. The clinician will bring flexible, comprehensive, and personalized health evaluations to people's homes.

Requirements

• Active, unrestricted license(s) in coverage area(s)
• Board certification (required)
• Able to cover the following primary locations: Worcester, MA

Responsibilities

• Conduct in-home health evaluations, including medication and medical history review, physical evaluation, and diagnostic tests if ordered.
• Use an iPad and clinical workflow for each evaluation.
• Join a national network of clinicians dedicated to improving health outcomes.
• Participate in efforts to visit millions of people nationwide to provide connections to the right care.
• Work with Signify Health as a clinician colleague.
• Be flexible to travel locally and within licensed states.
• Conduct virtual visits as needed.
• Provide all Signify Health services including Diagnostic & Preventive Services products (i.e., spirometry, DEE, etc.).
