About The Position

Signify Health, part of CVS Health®, is seeking a Part Time Clinician (Nurse Practitioner or Physician Assistant) to provide In-Home Health Evaluations. This role involves engaging with individuals in their homes to bridge gaps in care and build trusted relationships. The goal is to provide flexible, comprehensive, and personalized health evaluations. The visit, lasting approximately 45 minutes, includes a medication and medical history review, a physical evaluation, and potentially diagnostic tests if ordered by the health plan. Clinicians will use an iPad with an intuitive workflow and are not required to prescribe medication, order lab tests, or alter current treatment regimens. The primary workplace is the private residence of health plan members, which can vary in conditions and may include household pets, secondhand smoke, and other airborne allergens.

Responsibilities

Conduct In-Home Health Evaluations.
Engage with individuals in their homes to bridge gaps in care.
Build trusted relationships to make people healthier.
Provide flexible, comprehensive, and personalized health evaluations.
Conduct medication and medical history reviews.
Perform physical evaluations.
Perform additional diagnostic tests as ordered by the person’s health plan (e.g., diabetic eye exams, spirometry).
Use an iPad and an intuitive clinical workflow for each evaluation.

Conduct virtual visits as needed.
Provide all Signify Health services as indicated including Diagnostic & Preventive Services products (i.e., spirometry, DEE, bone density screening, etc.).
