About The Position

Theoria Medical is seeking a Nurse Practitioner (NP) or Physician Assistant (PA) to provide high-quality, patient-centered care to residents in skilled nursing facilities in Cumberland, WI. This role focuses on building relationships, ensuring consistency, and making a lasting difference for vulnerable populations. Theoria Medical offers a unique blend of medical excellence and technological advancement, serving the post-acute sector. The position offers flexible scheduling and autonomy, supported by intuitive technology and a collaborative clinical team. The role involves daily rounding, prescribing medications and treatment plans, addressing acute/emergent issues, and communicating with facility nursing staff. The position requires reliable on-site attendance for scheduled shifts and compliance with clinical standards, facility policies, and regulatory requirements.

Requirements

  • Active U.S. license
  • Board certified by ANCC, AANPCB, PNCB, or AACN (for Nurse Practitioners), or by NCCPA (for Physician Assistants)
  • Qualified specialties include but not limited to Internal Medicine, Family Medicine, Emergency Medicine, General Practice, Physical Medicine and Rehabilitation, and Geriatric Medicine.
  • Must be able to perform the essential functions of this position satisfactorily, with or without a reasonable accommodation.
  • Theoria Medical conducts criminal background checks and pre-employment drug testing on all candidates upon acceptance of contingent offer.
