About The Position

Signify Health, part of CVS Health®, is seeking a Full Time Clinician (Nurse Practitioner or Physician Assistant) to provide In-Home Health Evaluations. This role involves engaging with people in their homes to bridge gaps in care and build trusted relationships to make people healthier. The position brings flexible, comprehensive, and personalized health evaluations directly to individuals. The evaluations typically last about 45 minutes and include a medication and medical history review, a physical evaluation, and potentially diagnostic tests if ordered by the health plan. Clinicians use an iPad with an intuitive workflow and are not required to prescribe medication, order lab tests, or alter current treatment plans. The role involves joining a national network of clinicians focused on improving health outcomes, visiting millions of people nationwide, and fulfilling the requirements of the specified role. Flexibility for local travel and virtual visits is required, along with providing all Signify Health services, including Diagnostic & Preventive Services products.

Requirements

Active, unrestricted license(s) in coverage area(s)
Board certification (required)
Must be able to comfortably and safely perform duties in homes that may contain household pets, secondhand smoke and other potential airborne allergens or irritants.
