About The Position

Signify Health, part of CVS Health®, is seeking a Part Time Clinician (Nurse Practitioner or Physician Assistant) to provide In-Home Health Evaluations. This role involves engaging with people in their homes to help bridge gaps in care and build trusted relationships to improve health outcomes. The clinician will provide flexible, comprehensive, and personalized health evaluations. The visit, lasting about 45 minutes, includes a medication and medical history review, a physical evaluation, and potentially diagnostic tests. Clinicians use an iPad with an intuitive workflow and are not required to prescribe medicine, order lab tests, or alter current treatment regimens. The primary workplace is the private residence of health plan members, which can be diverse and may contain household pets, secondhand smoke, and other potential airborne allergens or irritants. Candidates must be able to perform duties safely in these environments.

Requirements

Active, unrestricted license(s) in coverage area(s).
Board certification (required).

Responsibilities

Conduct In-Home Health Evaluations.
Engage with people in their homes to help bridge gaps in care.
Build trusted relationships to make people healthier.
Provide flexible, comprehensive, and personalized health evaluations.
Conduct medication and medical history reviews.
Perform physical evaluations.
Conduct additional diagnostic tests if ordered by the person’s health plan (i.e., diabetic eye exams, spirometry, etc.).
Use an iPad and an intuitive clinical workflow for each evaluation.

Conduct virtual visits as needed.
Provide all Signify Health services as indicated including Diagnostic & Preventive Services products (i.e., spirometry, DEE, bone density screening, etc.).
