About The Position

This outpatient advanced practice provider role serves an underserved pediatric population in a school-based health center. The position is described as part of CHSN School-based Health and is split between Pleasant Hill Academy and Midway Elementary School. The APP provides care within the scope of the collaborating or supervising physician and in accordance with applicable law, professional standards, and Cincinnati Children’s policies. The schedule is full time, 40 hours per week, on a day shift; shift lengths are 4–8 hours, and call is required. The role includes assessing and diagnosing acute and chronic conditions, developing and monitoring individualized plans of care, ordering and monitoring appropriate treatments and diagnostic interventions, providing clinical consultation, and prescribing medications within the applicable scope of practice. The provider collaborates with patients, families, caregivers, and healthcare team members to coordinate care, support self-management, make referrals, and evaluate outcomes. The role also involves maintaining clinical competency, contributing to quality improvement and professional development, and advocating for pediatric patients and their families.

Requirements

Experienced Nurse Practitioner and/or Physician Assistant.
Certification as FNP, PNP-Primary Care, or NCCPA.
Master’s degree in a related field, or Doctor of Nursing Practice degree.
Licensure and certification in accordance with Cincinnati Children’s Delineation of Privileges; clinical license, certification, or training may be required.
The scope of the subspecialty role must not exceed the scope of the collaborating or supervising physician.

Nice To Haves

Specialty experience, preferably pediatric primary care.
Multi-state licensure in addition to Ohio licensure may be required based on subspecialty.

Responsibilities

Take patient histories and perform physical examinations for patients presenting for care within the area of expertise.
Assess and diagnose acute and chronic illnesses; formulate, document, and monitor treatment plans.
Order and monitor therapeutic interventions, including diets, laboratory tests, radiological tests, and other diagnostic tests as appropriate.
Provide clinical consultation and order interdisciplinary consultations.
Prescribe medications in accordance with federal and state laws and rules and Cincinnati Children’s policies.
Maintain competency in subspecialty practice, skills, and procedures in accordance with Joint Commission focused and ongoing professional practice evaluation standards.
Develop measurable outcomes and individualized, comprehensive plans of care, involving patients and families when possible; prioritize, evaluate, and modify goals and plans as needed.
Consider risks and benefits, costs, clinical effectiveness, scientific evidence, clinical guidelines, and clinical expertise when planning interventions and outcomes.
Maintain documentation of outcomes, goals, and plans of care, updating it as needed.
Maintain professional behaviors aligned with core values and care standards; participate in quality improvement or research activities.
Pursue lifelong learning and professional development to improve clinical skills and role performance.
Serve as a preceptor, role model, or mentor and contribute to the professional development of peers and colleagues.
Integrate ethical considerations and research findings into practice; participate in committees or professional organizations, or contribute through writing, publishing, or presenting.
Advocate for pediatric patients and their families and provide leadership influencing healthcare or advanced practice nursing practice and policy.
Promote self-management and family-centered care, use teaching and other strategies to promote health and safety, and provide leadership in care management.
Document and communicate with healthcare team members to support integrated pediatric care; delegate appropriate monitoring, assessment, and interventions within the caregiver’s scope of practice.
Provide consultation, make appropriate referrals, discuss recommendations with patients and families, and order or perform appropriate treatments, therapies, and procedures.
Communicate diagnostic and laboratory results and explain the benefits, limitations, and potential adverse effects of proposed therapies to patients and families.
Systematically evaluate assessment data, diagnoses, outcomes, interventions, and care plans; involve patients, families, and healthcare providers in evaluation and document findings appropriately.
Use evaluation results to determine impact on patients, families, the organization, and the healthcare system, and make or recommend process or structural changes when appropriate.
