About The Position

Impact Health PC is seeking a compassionate and motivated Nurse Practitioner or Physician Assistant to join their growing clinical team. This role involves serving patients within skilled nursing care facilities in Lancaster, PA. Impact Health PC is a leading advanced practitioner group in skilled nursing facilities across the Northeast, dedicated to prioritizing patient care and specializing in Medical, Psychiatric Services, Wound Care, and Palliative Care. The position offers a flexible Monday through Friday schedule with no nights, weekends, or holidays, and provides a collaborative environment that values expertise and independence.

Requirements

Active license as a Nurse Practitioner (NP) or Physician Assistant (PA)
Minimum 2 years of post-certification experience
Proficient with EHRs and clinical technology
Strong communicator with a collaborative mindset

Nice To Haves

Experience in SNFs, hospitalist roles, emergency departments, or ICU preferred

Responsibilities

Deliver person-centered care using evidence-based practices
Collaborate with interdisciplinary facility teams
Conduct assessments, develop treatment plans, and manage follow-up care
Educate patients, families, and staff
Complete timely documentation (within 24 hours)
Ensure compliance with state regulations in SNF settings

Earning potential: $110,000–$200,000 (salary based on experience + easily achieved monthly bonuses)
