About The Position

Relive Health provides personalized, proactive care through comprehensive testing, clinical evaluation, and individualized treatment planning. Our North Wales clinic offers hormone therapy, medical weight management, functional health, peptide therapies, IV therapy, and aesthetic services. We are seeking a clinically confident Nurse Practitioner or Physician Assistant with hands-on experience prescribing and managing hormone therapy. This position primarily focuses on HRT/TRT, women’s hormone health, metabolic health, medical weight management, and ongoing wellness care. The ideal candidate is comfortable evaluating patients, interpreting laboratory results, developing individualized treatment plans, and monitoring treatment response over time. Experience managing HRT/TRT is required, with the ability to evaluate and manage hormone-related concerns in both men and women. Medical weight-management experience is strongly preferred. Additional experience with peptide therapies or functional/integrative medicine is valued. You will bring an established clinical foundation and receive Relive-specific onboarding, clinical resources, continuing education, and collaborative medical guidance. This is an onsite position at our North Wales, Pennsylvania clinic.

Requirements

  • Graduate of an accredited Nurse Practitioner or Physician Assistant program
  • Active, unrestricted Pennsylvania credentials appropriate to the role and current national certification
  • Hands-on experience prescribing and providing ongoing management of HRT/TRT
  • Clinical competence in evaluating and managing hormone-related concerns in both men and women
  • Experience interpreting hormone and metabolic laboratory results and using findings to guide treatment and monitoring
  • Strong understanding of patient selection, contraindications, informed consent, adverse effects, and follow-up requirements
  • Required prescribing authorization and practice-specific collaborative or supervisory arrangements in place before providing care
  • Ability to obtain and maintain DEA registration as required for assigned prescribing responsibilities
  • Strong clinical judgment, patient communication, teamwork, and organizational skills
  • Comfortable using electronic health records, laboratory portals, pharmacy systems, and electronic prescribing platforms
  • Ability to maintain a dependable onsite schedule at the North Wales clinic

Nice To Haves

  • Three or more years of relevant experience practicing as an NP or PA
  • Experience prescribing and managing medical weight-loss therapies
  • Additional training or experience in menopause care, metabolic health, or functional/integrative medicine
  • Experience evaluating and managing appropriate peptide therapies
  • Experience supporting long-term patient engagement and continuity of care
  • Aesthetic injectable experience is welcome but is not a substitute for hormone-management experience

Responsibilities

  • Conduct comprehensive patient evaluations, physical examinations, laboratory reviews, and follow-up visits
  • Assess symptoms, medical history, medications, lifestyle, treatment goals, and relevant risk factors
  • Interpret hormone, metabolic, and other clinically indicated laboratory testing
  • Evaluate, prescribe, and manage HRT/TRT for appropriately selected patients
  • Evaluate and manage women experiencing perimenopause, menopause, and other hormone-related concerns
  • Develop individualized, evidence-informed treatment plans within your clinical scope and expertise
  • Prescribe and manage medical weight-loss treatments, including GLP-1 medications when clinically appropriate
  • Evaluate the evidence, patient suitability, and applicable prescribing requirements for peptide therapies offered by the practice
  • Monitor symptoms, laboratory findings, treatment response, and adverse effects, adjusting care as appropriate
  • Explain treatment options, risks, benefits, alternatives, expected outcomes, and follow-up requirements
  • Support continuity of care through timely follow-up, patient communication, and coordination with the clinical team
  • Recognize findings requiring additional evaluation, referral, or escalation
  • Collaborate with physicians, clinical staff, and pharmacy partners
  • Complete accurate and timely clinical documentation
  • Maintain patient privacy, safety, infection-control, and applicable compliance standards
  • Participate in continuing education and Relive training
