Responsibilities
- Primary responsibility for primary care services, acute care services, and routine monitoring of chronic conditions.
- Implementing best practices to avoid hospitalizations and emergency department visits and maximize functional improvements and quality of life.
- Collaborate and provide clinical support to nurse practitioners, typically one per facility, empowering them with guidance and sharing medical expertise.
- Ensure regulatory compliance.
