About The Position

Ennoble Care is seeking an experienced Nurse Practitioner or Physician Assistant to provide primary and palliative care to homebound patients in the Essex County, New Jersey region. The clinician applies medical techniques and principles under physician supervision and cares for patients with chronic diseases. Ennoble Care provides mobile primary care, palliative care, and hospice services, with clinicians visiting patients at home and offering programs such as remote patient monitoring, behavioral health management, and chronic care management.

Requirements

• Board certified and hold valid Nurse Practitioner or Physician Assistant certification in New Jersey.
• Strong clinical skills and the ability to confidently and accurately treat, assess, and diagnose.
• Positive demeanor and strong interpersonal skills.
• Ability to problem-solve confidently and work independently.
• One to two years of primary care experience with geriatric patients.

Nice To Haves

• Hospice experience is highly preferred but not required.

Responsibilities

• Provide high-quality primary and palliative care to patients in and around Essex County, New Jersey.
• Assess patients’ health through physical examinations, obtaining and updating medical histories, and developing care plans.
• Communicate effectively with office staff and clinical operations using internal platforms.
• Provide patient-centered care to a panel of patients.
• Attend quarterly medical staff meetings.
• Develop clinical impressions, diagnoses, and treatment plans.
• Build trust and consult with patients, families, and caregivers during visits.
• Order diagnostic testing when medically necessary.
• Prescribe medications and supplies and provide appropriate medically necessary interventions.
