About The Position

The Program of All-Inclusive Care for the Elderly (PACE) offers a variety of services, with many of them provided onsite at a PACE Center. PACE is an alternative to nursing home care and – through an interdisciplinary care team (IDT) of physicians, nurse practitioners, nurses, social workers, therapists, van drivers, and aides – coordinates and provides preventive, primary, acute, and long-term care services, so older individuals can continue living in the community. In this role, you'll provide compassionate, high-quality primary care to Senior Care Partners PACE participants in collaboration with a licensed physician, supporting their health, independence, and well-being.

Requirements

  • Bachelor’s degree required, Master’s degree preferred.
  • Must be a graduate of an accredited Nurse Practitioner educational program.
  • Must be licensed as a Registered Nurse in the State of Michigan.
  • Must be licensed as a Nurse Practitioner in the State of Michigan.
  • Minimum of 3 years of experience as a PA/NP
  • Minimum of one year of experience with frail elderly population.
  • Only act within the scope of his or her authority to practice.
  • Meet a standardized set of competencies for the specific position description established by Senior Care Partners PACE and approved by CMS before working independently.
  • Must be knowledgeable of current medical practices and procedures, as well as laws, regulations, and guidelines that pertain to long-term care.
  • Must be willing to seek out new methods and principles and be willing to incorporate them into existing medical practices.
  • Must have reliable transportation, current valid Michigan driver’s license and car insurance.
  • Must be 18 years of age or older.
  • Must be fully vaccinated, or willing to be fully vaccinated against the following communicable diseases: Influenza, MMR, Varicella, Tdap/Td, Covid-19 and Hepatitis B.
  • Must be fully vaccinated or willing to become vaccinated against: Influenza, MMR, Varicella, Tdap/Td, COVID-19, and Hepatitis B

Nice To Haves

  • Minimum of one year home care experience preferred.
  • Minimum of one-year wound care experience preferred.

Responsibilities

  • Performs comprehensive history and physicals for new referrals.
  • Performs interval history and physicals on established participants, completed quarterly or as indicated.
  • Evaluates and treats participants with episodic illnesses.
  • Assists the IDT in formulating a plan for the coordination of 24-hour care.
