About The Position

We are seeking a full-time Advanced Practice Provider (Nurse Practitioner or Physician Assistant) to deliver high-quality Primary Care within a Value-Based Care (VBC) model. This role is ideal for an APP experienced in managing chronic conditions, improving outcomes, and supporting population health initiatives through coordinated, proactive care. The Nurse Practitioner role involves providing advanced healthcare services, diagnosing illnesses, prescribing medications, and collaborating with interdisciplinary teams to deliver holistic patient-centered care. With expertise in primary and specialty care, Nurse Practitioners play a vital role in improving health outcomes and enhancing the quality of life for patients.

Requirements

  • Nurse Practitioner and Registered Nurse licenses in the state of Florida.
  • National board certification as a Nurse Practitioner from an accredited agency.
  • Master’s in nursing required.
  • Two or more years of clinical experience as a nursing practitioner at a physician practice or in a hospital.
  • Prior Value-Based Care experience, including experience with quality measures, risk-based/panel management, care coordination, outcomes-based care delivery, and documentation practices that support accurate clinical capture and continuity of care.
  • Strong foundation in Primary Care (Internal Medicine and/or Family Medicine preferred).
  • Comfort managing medically complex, chronically ill patient populations.
  • Collaborates with nurse practitioners, registered nurses, doctors, and other medical professionals.
  • Problem-solves well independently and as part of a team.
  • Develops strong relationships with patients and their families and has a stellar bedside manner.

Nice To Haves

  • Board certification as a Nurse Practitioner in the specialty is preferred.
  • Doctorate of Nursing preferred.
  • Preferred Clinical Experience (Renal/Nephrology) - Renal and/or nephrology experience is highly preferred, including familiarity with CKD management, hypertension, diabetes-related renal disease, electrolyte issues, and coordination of care for patients with renal impairment (including dialysis coordination when applicable).
  • Bilingual in English & Spanish preferred.
  • Communicates effectively, speaking multiple languages is preferred.

Responsibilities

  • Maintains confidentiality of all patient records.
  • Checks in with clients about test results and prognoses.
  • Advises patients regarding prescriptions and treatment plans.
  • Follows all federal, state, and nursing guidelines, regulations, and standards.
  • Orders and interprets diagnostic tests, including x-rays, blood work, and scans.
  • Refers patients to specialists if necessary and analyzes medical records and reports.
  • Updates patient charts and uploads to Electronic Health Record (EHR) software.
  • Works with a team of nurse practitioners, registered nurses, and doctors to provide quality patient care.
  • Examines patients, including discussing medical history and symptoms and performing physicals.
