Nurse Practitioner (Value-Based Care)
About The Position
The Nurse Practitioner role involves providing advanced healthcare services, diagnosing illnesses, prescribing medications, and collaborating with interdisciplinary teams to deliver holistic patient-centered care. With expertise in primary and specialty care, Nurse Practitioners play a vital role in improving health outcomes and enhancing the quality of life for patients. This is a 100% outpatient primary care clinic setting seeing patients 18+ with a dedicated Medical Assistant and additional clinic support team. The role operates on a Monday – Friday schedule with the possibility of extended hours, and telehealth is available for patients.
Requirements
- Nurse Practitioner and Registered Nurse licenses in the state of Texas.
- National board certification as a Nurse Practitioner from an accredited agency.
- Master’s in nursing required.
- Two or more years of clinical experience as a Nurse Practitioner in a physician practice, clinic or hospital setting.
Nice To Haves
- Board certification as a Nurse Practitioner in the specialty is preferred.
- Doctorate of Nursing preferred.
- Bilingual in English & Spanish preferred.
Responsibilities
- Maintains confidentiality of all patient records.
- Checks in with clients about test results and prognoses.
- Advises patients regarding prescriptions and treatment plans.
- Follows all federal, state, and nursing guidelines, regulations, and standards.
- Orders and interprets diagnostic tests, including x-rays, blood work, and scans.
- Refers patients to specialists if necessary and analyzes medical records and reports.
- Updates patient charts and uploads to Electronic Health Record (EHR) software.
- Works with a team of nurse practitioners, registered nurses, and doctors to provide quality patient care.
- Examines patients, including discussing medical history and symptoms and performing physicals.
