Nurse Practitioner or Physician Assistant - Senior Services, Home Visit Programs
About The Position
Primary Care in the Rochester region is seeking a Nurse Practitioner or Physician Assistant to work as part of a multidisciplinary team to provide care to those living within our community through the Senior Service Home Visiting Programs. Within in this position the NP/PA will work collaboratively with internal medicine physicians to provide holistic care including acute, chronic, preventive and palliative care services to patients in private homes and senior living centers. The provision of care includes but is not limited to comprehensive evaluation, ordering and interpreting diagnostic tests, prescription of pharmacologic agents and treatments and non-pharmacologic therapies, and counseling patients.
Requirements
- Experience in Palliative Care, Primary care and/or Geriatrics.
Responsibilities
- Provide holistic care including acute, chronic, preventive and palliative care services to patients in private homes and senior living centers.
- Conduct comprehensive evaluations.
- Order and interpret diagnostic tests.
- Prescribe pharmacologic agents and treatments.
- Prescribe non-pharmacologic therapies.
- Counsel patients.
