About The Position
Theoria Medical is seeking an Advanced Practice Provider to serve local skilled nursing facilities within a close-knit, mission-driven care community. This role focuses on providing high-quality, patient-centered care to residents, including daily rounding, prescribing medications and treatment plans, and addressing acute/emergent issues. The position emphasizes building trust and strengthening continuity of care for vulnerable populations, offering flexible scheduling and autonomy supported by technology and a collaborative team.
Requirements
- Active U.S. license
- Board-certified by ANCC, AANPCB, PNCB, or AACN (for Nurse Practitioners), or by NCCPA (for Physician Assistants)
- Qualified specialties include, but are not limited to, Internal Medicine, Family Medicine, Emergency Medicine, General Practice, Physical Medicine and Rehabilitation, and Geriatric Medicine.
- Must be able to perform the essential functions of this position satisfactorily, with or without a reasonable accommodation.
Responsibilities
- Provide high-quality, patient-centered care to residents in skilled nursing facilities
- Conduct daily rounding, prescribe medications/treatment plans, and address acute/emergent issues
- Communicate with facility nursing staff via phone or secure messaging as needed
- Maintain reliable on-site attendance for scheduled shifts
- Ensure compliance with clinical standards, facility policies, and regulatory requirements
