About The Position

Sutter Senior Care is seeking a full-time Nurse Practitioner or Physician Assistant to join our geriatrics-focused primary care team serving complex Medicare patients. This role operates within a collaborative, team-based care model, working in a dyad partnership with a geriatric physician. The focus is on comprehensive, relationship-based care for older adults with complex medical and social needs. The position involves a blend of direct patient care (~60%) and care management (~40%), including panel management and care coordination. Opportunities for in-clinic, home-based, and virtual care delivery exist, with interdisciplinary collaboration with RNs, social workers, pharmacists, and specialists. Requirements

Current California State NP or PA License
CAPA Controlled Substance Certification (for Physician Assistants only)
Current Nurse Practitioner Furnishing Number (for Nurse Practitioners only)
Nurse Practitioner furnishing number of NP’s
Active DEA registration
Experience in primary care, geriatrics, or value-based care models

Nice To Haves

Minimum 2–3 years of clinical experience preferred
Geriatrics or complex care experience strongly preferred
Experience with high-risk senior populations or panel-based care
Familiarity with Epic or other EHR systems
Experience with care management, transitions of care, or home-based care delivery

Responsibilities

Provide comprehensive geriatric assessments, diagnoses, and treatment plans
Manage acute, chronic, and complex medical conditions, including multimorbidity
Deliver preventive care, screenings, and medication management (including polypharmacy monitoring)
Perform in-basket management, including patient communications, results review, and follow-up
Support transitions of care (hospital/SNF follow-up)
Lead proactive panel management and care gap closure efforts
