About The Position

Sutter Senior Care is seeking a full-time Nurse Practitioner or Physician Assistant to join our geriatrics-focused primary care team serving complex Medicare patients. This role operates within a collaborative, team-based care model, working in a dyad partnership with a geriatric physician. The focus is on comprehensive, relationship-based care for older adults with complex medical and social needs. The position involves a blend of direct patient care (~60%) and care management (~40%), including panel management and care coordination. Opportunities for in-clinic, home-based, and virtual care delivery exist, with interdisciplinary collaboration with RNs, social workers, pharmacists, and specialists. Requirements

  • Current California State NP or PA License
  • CAPA Controlled Substance Certification (for Physician Assistants only)
  • Current Nurse Practitioner Furnishing Number (for Nurse Practitioners only)
  • Nurse Practitioner furnishing number of NP’s
  • Active DEA registration
  • Experience in primary care, geriatrics, or value-based care models

Nice To Haves

  • Minimum 2–3 years of clinical experience preferred
  • Geriatrics or complex care experience strongly preferred
  • Experience with high-risk senior populations or panel-based care
  • Familiarity with Epic or other EHR systems
  • Experience with care management, transitions of care, or home-based care delivery

Responsibilities

  • Provide comprehensive geriatric assessments, diagnoses, and treatment plans
  • Manage acute, chronic, and complex medical conditions, including multimorbidity
  • Deliver preventive care, screenings, and medication management (including polypharmacy monitoring)
  • Perform in-basket management, including patient communications, results review, and follow-up
  • Support transitions of care (hospital/SNF follow-up)
  • Lead proactive panel management and care gap closure efforts
