This position involves rotating through all inpatient Pulmonary Units, including Base Transitional Care Center (TCC), Liberty Transitional Care Center (TCC), and A7C (Complex Pulmonary). On the TCC units, the role serves patients from infant to adult who are dependent on positive pressure support (trach/ventilator dependent, non-invasive/BiPAP dependent). These patients often have complex disease processes affecting their airway, lungs, and respiratory mechanics, including conditions like Bronchopulmonary Dysplasia (BPD), neuromuscular diseases (e.g., Duchenne’s Muscular dystrophy, Spinal Muscular Atrophy), and restrictive lung disease due to scoliosis or chest wall abnormalities. A key responsibility is training families on home ventilator use, tracheostomy tube care, GTube care, and emergency preparedness for technology-dependent loved ones. On A7C, the focus is on managing complex pulmonary conditions such as Cystic Fibrosis, Asthma, Obstructive Sleep Apnea, Rare Lung Disease, and patients undergoing or having received a lung transplant. Across all three units, families are trained on providing their child’s care needs at home, including education on home ventilators, tracheostomies, airway clearance modalities, medications, feeding regimens, and emergency preparedness. While the role is within pulmonary medicine, patients are complex and often have multiple diagnoses across various body systems, offering a broad-spectrum experience with a pulmonary focus. The scope for this subspecialty role will not exceed the scope of the collaborating or supervising MD.
