About The Position

Hospital Medicine APPs practice at the top of their license with substantial autonomy and serve as core clinical leaders within interdisciplinary teams. Depending on assignment, care is delivered through a combination of in-person and virtual workflows for medically complex adult patients. In the Care Transition Unit (CTU), APPs independently manage high-volume, rapid-turnover patients, focusing on diagnostic clarity, evidence-based protocols, timely disposition, and efficient movement between emergency, observation, and inpatient pathways.

Requirements

  • Practice at the top of their license with substantial autonomy

  • Focus on diagnostic clarity, evidence-based protocols, timely disposition, and efficient movement between emergency, observation, and inpatient pathways
  • Admit, transfer, and manage hospitalized and observation patients
  • Perform comprehensive histories, physical examinations, and daily clinical assessments
  • Order, interpret, and act on diagnostic and therapeutic studies
  • Formulate diagnoses and manage acute and chronic medical conditions
  • Lead inpatient and observation care plans, including escalation and de-escalation decisions
  • Coordinate care with consultants, nursing, and ancillary services
  • Educate patients and caregivers and lead discharge planning
  • Prescribe medications and perform minor procedures as indicated
  • Ensure age-appropriate, patient-centered, and evidence-based care
