About The Position

This role is for a Nurse Practitioner (NP) or Physician Assistant (PA) in Family Medicine, with practice locations within the Baldwin Building, NW Clinic, NE Clinic, SE Clinic, and Kasson Clinic in Rochester. The specific practice location will be determined by practice needs at the time of hire. The NP or PA will function within a care team setting, collaborating with physicians and allied health staff. Responsibilities include population management strategies to enhance chronic illness outcomes. Patient care encompasses comprehensive evaluations, ordering and interpreting diagnostic tests, prescribing medications and treatments, utilizing non-pharmacologic therapies, and patient counseling.

Requirements

• Nurse Practitioner: Masters prepared graduate of an accredited school of nursing and Nurse Practitioner program.
• Current Minnesota RN and APRN license and certification as an FNP.
• Physician Assistant: Graduate of accredited Physician Assistant program with current PA certification by NCCPA and registration as a PA by the Minnesota State Board of Medical Practice.
• BLS certification and DEA registration required.
• Comfortable with autonomy and independent decision making.
• Possess excellent communication skills.

Nice To Haves

• DNP is preferred for Nurse Practitioners.
• DMSc is preferred for Physician Assistants.
• NP or PA provider experience preferred.
• Experience in primary care preferred.
• Previous experience working with pediatric as well as adult patients preferred.
• Team player, comfortable seeing patients of all ages.

Responsibilities

• Work in a care team setting in collaboration with other team members including physicians and other allied health staff.
• Be involved in population management strategies to improve chronic illness outcomes.
• Provide comprehensive evaluation, order and interpret diagnostic tests, prescribe pharmacologic agents and treatments, non-pharmacologic therapies, and counsel patients.
• See patients for chronic disease management, preventive care, health promotion and acute care.
