This is a full-time position for an Advanced Practice Clinician (Nurse Practitioner or Physician Assistant) specializing in Advanced Heart Failure Therapies within an inpatient hospital setting. The role involves evaluating, diagnosing, treating, and managing acute, critical, and chronic patient illnesses and diseases. The clinician will obtain medical histories, conduct physical examinations, assess and determine differential diagnoses, and prescribe medications, therapies, and treatments. Responsibilities include interpreting patient data, performing minor surgeries and routine procedures, consulting with physicians, and ordering/interpreting diagnostic studies. The role also involves adjusting patient care plans, counseling and educating patients on health, providing care coordination (including managing ancillary services, pre-operative plans, discharge planning, referrals, and medical devices), and prescribing pharmacologic and non-pharmacologic therapies. The clinician will also prepare patient care plans and summaries, maintain records, and provide clinical direction to nursing staff. The position requires the ability to provide age-appropriate care and demonstrate knowledge of growth and development principles.
