About The Position
The Heart Failure Integrated Practice Unit (IPU) serves as the foundation of a nationally recognized heart failure program, grounded in clinical excellence, innovation, and patient-centered care. Our program has achieved Heart Failure Certification through DNV and Gold Plus Status from the American Heart Association, reflecting our commitment to evidence-based standards and continuous improvement in quality metrics and guideline-directed care. Our multidisciplinary team advances patient outcomes through comprehensive education, individualized transition planning, and long-term disease management, aiming to reduce readmissions, improve symptom control, and enhance the quality of life for patients with heart failure. The heart failure nurse is central to this work, providing expert nursing care by utilizing the nursing process to assess, plan, implement, and evaluate patient-centered care across the continuum. Practicing within the framework of organizational policies and procedures, the heart failure nurse demonstrates professional accountability, clinical leadership, and a commitment to ongoing growth. This role is responsible for upholding standards of nursing practice, coordinating patient care activities, and collaborating with the interdisciplinary team to ensure safe, effective, and high-quality care. Through clinical expertise, coordination of care, and patient advocacy, the heart failure nurse contributes significantly to the success of the program and the delivery of exceptional outcomes for our patients.
Requirements
- Graduate of a nursing program
- New Graduates of a nursing program eligible
- Current License in the State of Georgia as a Registered Nurse or NLC/eNLC Multistate License.
- BLS certification required.
Nice To Haves
- Advanced certification in field of specialty, if applicable.
- Bachelor's degree preferred.
- Nursing Experience in Hospital Setting Preferred.
