This role involves performing a variety of functions and technical procedures necessary for patient admission, assessment, diagnosis, treatment, follow-up, and outcome evaluation. The position also participates in performance improvement, research, and educational activities. The School Based Health Center (SBHC) Advanced Clinical Provider (ACP) provides core clinical services including primary and preventive health care, diagnosis and treatment of medical conditions, management of chronic conditions, age-appropriate reproductive health care, HEADDSSS assessments, risk behavior assessments, immunizations, population and individual-based needs assessments, and on-site routine management of chronic conditions. Medications are dispensed for most medical needs, and prescriptions may be provided. Laboratory evaluations are available, including point-of-care testing. Core administrative activities include ensuring regulatory and quality assurance protocols are operational, maintaining communication logs, managing logs for laboratory and pharmacy, maintaining regulatory documentation for equipment, and overseeing quality improvement assessments. The ACP collaborates with the school, orders supplies, and oversees staff orientations, meetings, and competencies. The Hospital NP develops, implements, and evaluates clinical collaborative practice agreements, conducts health assessments, initiates referrals, and prescribes medical regimes. This role also involves patient/family education, participation in team meetings, serving on committees, acting as a liaison and advocate, identifying quality improvement issues, participating in research studies, maintaining current knowledge, and acting as a resource for staff.
