About The Position

This role involves performing a variety of functions and technical procedures necessary for patient admission, assessment, diagnosis, treatment, follow-up, and outcome evaluation. The position also participates in performance improvement, research, and educational activities. The School Based Health Center (SBHC) Advanced Clinical Provider (ACP) provides core clinical services including primary and preventive health care, diagnosis and treatment of medical conditions, management of chronic conditions, age-appropriate reproductive health care, HEADDSSS assessments, risk behavior assessments, immunizations, population and individual-based needs assessments, and on-site routine management of chronic conditions. Medications are dispensed for most medical needs, and prescriptions may be provided. Laboratory evaluations are available, including point-of-care testing. Core administrative activities include ensuring regulatory and quality assurance protocols are operational, maintaining communication logs, managing logs for laboratory and pharmacy, maintaining regulatory documentation for equipment, and overseeing quality improvement assessments. The ACP collaborates with the school, orders supplies, and oversees staff orientations, meetings, and competencies. The Hospital NP develops, implements, and evaluates clinical collaborative practice agreements, conducts health assessments, initiates referrals, and prescribes medical regimes. This role also involves patient/family education, participation in team meetings, serving on committees, acting as a liaison and advocate, identifying quality improvement issues, participating in research studies, maintaining current knowledge, and acting as a resource for staff.

Requirements

Master’s Degree in a Nurse Practitioner Program and National Board Certification in specialty area required.
Current license to practice as a Nurse Practitioner in New York State, and current license to practice as a Registered Professional Nurse in New York State, required.
Bachelor’s Degree, required.
Graduate of an accredited (Accreditation Review Committee for Physician Assistant Programs ARC-PA) program, required.
Current license to practice as a Physician Assistant (PA) by the New York State Department of Education, required.
Current NCCPA Certification from the National Commission on the Certification of Physician Assistants (NCCPA), required.
Current BLS certification, required.
Has a collaborative practice agreement with designated protocols filed with NYS Department of Education OR collaborative relationship attestation as per the Nurse Practitioner Modernization Act 2015.
Current BLS certification, required.

Responsibilities

Takes initial patient histories and performs physical examinations.
Initiates, documents, and communicates the plan of care and follows up to ensure complete screening and preparation of patients.
Performs daily patient rounds.
Writes orders for medications, laboratory work, and diagnostic tests.
Interprets laboratory and test results.
Confers with attending physicians, residents, nursing staff, and/or other care providers to ensure optimum quality of patient care.
Upon discharge, notes discharge summaries, prescriptions, and any referrals on patient charts.
Informs patients of the necessary post-discharge care instructions.
Provides primary and preventive health care, diagnosis and treatment of medical conditions and management of chronic conditions.
Provides age-appropriate reproductive health care.
Conducts HEADDSSS and an assessment of risk behaviors.
Manages immunizations consent, provision and documentation.
Performs population and individual-based assessment of needs.
Provides on-site routine management of chronic conditions (asthma, diabetes, etc.), provided in consultation with the child's outside primary care provider or specialist as appropriate.
Dispenses medications for most medical needs and provides prescriptions for acute or chronic conditions.
Performs laboratory evaluations as needed for both reproductive and general medical health needs, including point of care testing.
Ensures that regulatory and quality assurance protocols are operational for their SBHC and that day-to-day operations are as designated in the Principals and Guidelines for the NYSDOH.
Conducts regulatory review of charts, maintenance of communication logs with community physicians and referrals, maintenance of logs for laboratory and pharmacy, maintenance of regulatory documentation for equipment including refrigerators and on-site laboratory equipment (urinalysis, glucometer, pregnancy testing and hemoglobin testing) as well as other nursing managed regulatory documentation.
Oversight of quality improvement (CQI) annual assessments in coordination with Medical Director and staff.
Works with the Operations Manager and LPN/MA to assist with ordering of medical, laboratory and pharmaceutical supplies.
Collaborates and communicates with school (Safety/BRT meetings, emergencies, school functions, custodians etc.).
Oversight of staff orientations, staff meetings and annual competencies.
Develops, implements, and evaluates the clinical collaborative practice agreements in the specific clinical area.
Conducts health assessments of patients/families with real or potential, acute, and/or chronic health problems.
Initiates referrals to specialty services and fosters continuity of care.
Initiates selected therapeutic and diagnostic procedures, consultation and laboratory examinations.
Prescribes and revises medical regimes.
Interprets findings and plans of care according to collaborative practice agreements and protocols in designated areas.
Provides patient/family education and counseling in the areas of health promotion, maintenance, and disease prevention.
Participates in team meetings and conferences to enhance an interdisciplinary approach to primary health care delivery.
Serves on departmental and multidisciplinary committees, which influence or determine policies affecting nursing practice.
Serves as liaison between patient and community and acts as health care advocate.
Identifies quality improvement issues and develops unit-based quality improvement programs, as well as standards of care and practice.
Participates in approved medical and nursing research studies.
Maintains a current level of knowledge relative to professional practice.
Acts as resource to staff and implements educational programs.
Participates in the evaluation of the cost effectiveness of services provided.
