About The Position

As an Oncology Navigation Nurse Practitioner at Thyme Care, you will be a vital clinical team member dedicated to supporting members through their cancer journey. Your role involves evaluating and managing members with urgent symptoms or treatment complications via telemedicine, developing comprehensive care plans in collaboration with other team members, and overseeing interdisciplinary case conferences. You will assess symptoms, create care plans, triage patients, educate them, and ensure follow-up with their primary treating clinicians. Additionally, you will conduct annual clinical intake visits, review medical records, provide clinical insights, and maintain high-quality documentation. You may also be involved in holiday, night, and weekend emergency coverage. All patient interactions are virtual.

Requirements

  • At least 3 years of NP experience.
  • At least 2 years in adult oncology with urgent care responsibilities.
  • At least one year of telehealth experience.
  • Unrestricted RN license.
  • Willingness to obtain additional state licenses as needed.
  • Compact RN license.
  • Unrestricted Advanced Practice license.
  • Skilled in juggling multiple tasks and working under pressure without sacrificing organization.
  • Effective listener and communicator; builds rapport and excellent working relationships.
  • Comfort with ambiguity and rapid changes in a fast-paced startup environment.
  • Desire to learn how to use new technologies.
  • Experience with video chatting, Google Suite, Slack, electronic health records or comfort in using and learning new technology.
  • Ability to identify priorities and take action to immediately address urgent and important needs.

Responsibilities

  • Evaluate and manage members with urgent symptoms, treatment complications, or exacerbations of medical conditions using a telemedicine platform.
  • Assess presenting symptoms, develop comprehensive care plans, triage to the appropriate level of care, educate patients, and ensure follow-up with relevant primary treating clinicians.
  • Complete annual clinical intake visits for select members.
  • Develop care plans in collaboration with onboarding RNs to address clinical and non-clinical needs and barriers, prioritizing members' goals.
  • Review available medical records, work with external providers, and give clinical insights to the care team.
  • Adhere to best documentation practices to drive high-quality care.
  • Be responsible for the oversight and direction of interdisciplinary case conferences for members.
  • Provide clinical oversight to the case management team and direct clinical care as needed.
  • Participate in holiday, night, and weekend emergency coverage as needed.
