We are actively seeking a Hospice Nurse Practitioner who is ready to step into a high-impact, field-based role where your clinical judgment directly shapes comfort, dignity, and end-of-life experience for patients and families. Requirements
- Current STATE RN and Advanced Practice Nurse License
- Valid Driver’s License
- Valid Auto Insurance
- AHA CPR Certification
Nice To Haves
- HomeCare HomeBase experience preferred
Responsibilities
- Provide face-to-face care in patient homes and facilities
- Make real-time clinical assessments that guide hospice eligibility and palliative decisions
- Work directly with hospice physicians and attending providers to shape care plans
- Educate families during some of the most important conversations of their lives
- Support and elevate the interdisciplinary team with your clinical expertise
- Document and communicate findings that directly impact patient comfort and care decisions
