About The Position

Visiting Docs is seeking an experienced, dedicated Nurse Practitioner (NP) to provide high-quality, patient-centered primary care to residents of assisted living facilities throughout Queens, New York. We are looking for a provider who combines strong clinical judgment with outstanding customer service. Our approach emphasizes accessible, attentive care, prompt communication, and dependable follow-through to ensure an exceptional experience for residents, families, and facility staff.

Requirements

• Active, unrestricted New York State Nurse Practitioner and Registered Nurse licenses.
• Minimum of three years of primary care experience as a Nurse Practitioner.
• Active DEA registration and NPI.
• Active Medicare and Medicaid enrollment.
• Strong clinical assessment, diagnostic, and medication management skills.
• Excellent interpersonal communication, organizational, and time management skills.

Nice To Haves

• Experience with eClinicalWorks (eCW) preferred.
• Experience caring for geriatric or assisted living populations is a plus.

Responsibilities

• Provide comprehensive primary care, including the evaluation, diagnosis, and treatment of acute and chronic medical conditions.
• Conduct routine assessments, preventive screenings, and follow-up visits.
• Prescribe and manage medications and review laboratory, imaging, and diagnostic results.
• Coordinate care with specialists, facility staff, family members, and other healthcare professionals.
• Respond promptly to clinical concerns, patient needs, and facility communications.
• Maintain accurate, thorough, and timely documentation in the electronic medical record.
• Demonstrate professionalism, punctuality, accountability, and strong communication skills.
• Build positive, collaborative relationships with residents, families, and assisted living facility teams.
