About The Position

This Inpatient Palliative Care Nurse Practitioner role involves serving patients 7 days a week in a hospital environment. The position requires providing quality care to patients from diverse socioeconomic and cultural backgrounds, with no on-call duty. The Nurse Practitioner will be responsible for educating caregivers on various aspects of patient care, including disease education, advance care planning, medication administration, and end-of-life expectations. The role is full-time, 40 hours per week, with a schedule determined based on team needs after an orientation period. Daily productivity goals include seeing 6-8 patients, with 1-2 new consults and 5-6 follow-up visits. The role involves extensive coordination with community medical providers and other disciplines, and active participation in a daily Interdisciplinary Team (IDT) meeting and a monthly business meeting with the CMO.

Responsibilities

  • See patients in a hospital environment.
  • Provide quality care regardless of circumstance to a variety of different socioeconomic and cultural backgrounds.
  • Educate caregivers on how to care for their loved one, including disease education, advance care planning, how to administer medications, and what to expect at end-of-life.
  • Coordinate care between your patient’s community medical providers, and other disciplines to provide updates, obtain and place new orders.
  • Be part of an Interdisciplinary Team (IDT) consisting of a physician, nurse practitioner, registered nurse, and a medical social worker.
  • Meet each morning at 8:30 AM with the IDT to review patient assignments and discuss complex cases as needed.
  • Attend a monthly business meeting with the CMO.
  • Collaborate with inpatient hospice clinical liaisons to coordinate and facilitate patient care at end-of-life.
