The Nurse Practitioner (NP) is an advanced practice registered nurse who has acquired specialized knowledge and skills through completion of a nurse practitioner program. The practitioner has the credentials to function as an independent licensed provider. The practitioner has responsibility for assessment and management of patients including diagnostic and therapeutic interventions, and continuous implementation and evaluation of an appropriate plan of care. The Nurse Practitioner works in collaboration with the Heart Faiure physicians and nursing staff and the Center for Advanced Heart Disease faculty of cardiologists and is responsible to the BWH Heart Failure program. The Nurse Practitioner participates with coordination and transition of care of the patient across all phases of the hospital, post discharge, and longitudinal heart failure setting, and will work closely with all members of the cardiovascular care team and primary care providers to deliver the best possible comprehensive care to cardiovascular patients. This position has the unique opportunity to serve the Center for Advanced Heart Disease Community Outreach Program and will require travel to practices located in: Foxboro, Westwood, Faulkner site in Jamaica Plain, Main BWH Campus Boston, and other sites as needed.
