About The Position

Our Geriatrics and Palliative Medicine team provides an extensive network of resources for the comprehensive care of older adults, bringing together a wide range of services that provide easy access to high-quality care. We are currently seeking a certified Adult Nurse Practitioner (NP) for our Housecalls program, an innovative program that brings providers to the homes of elderly patients in the neighborhood of NewYork-Presbyterian/Weill Cornell Medical Center and delivers care for those unable to visit our providers onsite. In collaboration with attending physicians, the NP is responsible for the coordination and direct care of patients in an ambulatory geriatrics practice setting. This role is also responsible for: providing patient education, following programs' panel of patients, documenting all encounters in Epic-EMR, billing appropriate level of service, returning all calls to programs' patients/loved ones/other providers, and execution of all medication refills/requests. Schedule: Monday-Friday, 9 am – 5 pm, onsite with ability to do house calls

Requirements

  • Master's Degree in Nursing
  • Approximately 1 year of clinical experience as a certified Adult NP in an ambulatory and/or home care setting.
  • New graduates with related RN or NP training experience may be considered.
  • Graduate of an accredited school of nursing.
  • Excellent fund of knowledge in geriatrics and primary care.
  • Demonstrated ability to multi-task and prioritize in a fast-paced environment.
  • Skills in establishing and maintaining effective working relationships with patients, caregivers, an interprofessional team, and with collaborating community agencies.
  • Excellent communication skills and strong interpersonal skills.
  • Excellent teaching skills and receptivity to teaching interprofessional trainees.
  • NYS Registered Nurse Practitioner (NP) license and current NP registration
  • NYS Registered Professional Nurse license and current RN registration
  • Applicable National Board Certification (such as ANCC, PNCB, etc).
  • Current BLS (must be issued by American Heart Association)
  • Infection Control Certificate.
  • Current Narcotics DEA registration certificate

Responsibilities

  • Diagnoses and treats acute and chronic health conditions in a geriatric population.
  • Uses telephone medicine skills to triage phone calls and develop assessments and plans.
  • Performs comprehensive geriatric functional assessments.
  • Sees patients in-person for urgent visits within an office setting.
  • Writes prescriptions according to New York State and federal law.
  • Communicates with patient, patient’s family, and/or caregiver regarding treatment plan.
  • Identifies, evaluates and addresses disease prevention and health promotion opportunities.
  • Initiates goals of care and end-of-life discussions when appropriate.
  • Performs and/or coordinates testing (POC or lab draws) for warfarin patients.
  • Develops timely and appropriate warfarin dosing plans and communicates plan to patients and caregivers.
  • Responsible for timely documentation in the electronic health records and accurate coding.
  • Collaborates with community/social service agencies in the care of patients.
  • Attends and participates in team meetings.
  • Attends continuing medical education (CME) programs.
  • Teaches geriatrics medicine to medical students, medical residents, geriatrics fellows, geriatric psychiatry fellows, and other trainees.
