About The Position
Deliver personalized home-based primary care from a Brooklyn hub, focusing on homebound patients throughout NYC. A large multispecialty group in NYC is seeking a Nurse Practitioner (NP) to provide high-quality, patient-centered care in patients’ homes and nursing facilities. This role is ideal for providers who enjoy holistic care, chronic disease management, and direct patient engagement.
Requirements
- Master’s Degree from an accredited NP program (Family or Adult/Geriatrics focus)
- Current New York State NP license
- National Board Certification (AANP, ANCC, or ONCC)
- Bilingual in Russian or Chinese
- Experience with EMR/EHR systems preferred
- Strong clinical judgment, communication skills, and ability to work independently
Responsibilities
- Assess and manage urgent medical issues during visits
- Participate in community outreach and patient education initiatives
