About The Position

We are seeking a Nurse Practitioner – Family Practice to join our Community Health Center team. This role provides comprehensive primary care to patients across the lifespan, including evaluation, diagnosis, treatment, prescribing, medication monitoring, preventive care, and chronic disease management. You'll work as part of an integrated healthcare team committed to delivering compassionate, culturally responsive care and improving health outcomes for the communities we serve.

Requirements

  • Current Arizona license as a Certified Nurse Practitioner with a focus on Family/Individual across the lifespan.
  • Full prescribing authority.
  • Completion of an accredited/approved Nurse Practitioner program.
  • Board certification.
  • Ability to provide safe, high-quality patient care within professional scope of practice.
  • Strong verbal, written, and interpersonal communication skills.
  • Ability to collaborate effectively with physicians, providers, clinical staff, and other healthcare professionals.
  • Ability to provide culturally sensitive care to diverse populations.
  • Working knowledge of electronic health records and/or Windows-based systems.
  • Must have or be able to obtain a valid Arizona Fingerprint Clearance Card.
  • Must have reliable transportation, a valid driver's license, and proof of auto insurance.
  • BLS certification, or ability to obtain training within 10 business days of hire.
  • Must complete required health screenings and applicable immunization/titer or declination requirements.

Responsibilities

  • Provide comprehensive primary care to patients across the lifespan.
  • Conduct patient evaluations, obtain medical histories, and perform complete physical examinations.
  • Diagnose and treat chronic and episodic medical conditions.
  • Assess patients' responses to medications and adjust treatment as medically appropriate.
  • Prescribe medications and recommend appropriate treatments within your scope of practice and prescribing authority.
  • Order, interpret, and evaluate diagnostic tests to identify and assess patient healthcare needs.
  • Develop individualized treatment plans, goals, and prognoses.
  • Educate patients and families about preventive care, medical conditions, medications, and treatment plans.
  • Respond to patient questions regarding medications, refills, and general healthcare needs.
  • Communicate with pharmacies, laboratories, radiology departments, and external healthcare providers as needed.
  • Complete clinical documentation accurately and within required timelines.
  • Respond to electronic health record tasks and provider action queues within established timeframes.
  • Participate in multidisciplinary team meetings and case staffing.
  • Collaborate with clinical and behavioral health teams to support integrated care.
  • Follow applicable federal and Arizona regulations, quality standards, and patient safety requirements.
  • Contribute to quality improvement initiatives and clinical review processes.
