About The Position

Under the supervision of the Medical Director, the Nurse Practitioner at Kolmac Integrated Behavioral Health and all its subsidiaries is responsible for providing evidence-based, integrated treatment to our patients. The NP assesses, diagnoses and provides continuity care for patients with co-occurring mental health and substance use disorders. The role involves providing integrated behavioral health services virtually and in person, assessing, diagnosing, and treating adult patients with co-occurring SUD and Mental Health disorders, and collaborating with a treatment team. Responsibilities include timely medical record completion, ordering and interpreting diagnostic tests, designing and documenting treatment plans, performing invasive procedures when needed, and communicating the plan of care with patients, families, and other providers. The role also includes educational responsibilities for healthcare team members, functioning as a clinical resource, providing community education, and participating in mentoring and precepting activities. Additionally, the NP is responsible for coordinating nurse practitioner coverage, working with primary care providers to achieve service-specific outcome targets, participating in performance improvement and quality assurance activities, advising on policies, and supervising ancillary staff.

Requirements

Must not be in recovery while working with DEA regulated medications
Active CRNP-PMH license in good standing
Active or pending CDS & DEA
Microsoft Office proficient
Understanding of DSM-V and diagnosis codes
Organizational and time management skills
Writing and oral communication skills
At least 1 year experience working in SUD population.

Responsibilities

Providing integrated behavioral health services to patients both virtually and in person.
Assessing, diagnosing, and treating adult patients with co-occurring SUD and Mental Health disorders.
Establishes medical diagnoses based on history, assessment, and diagnostic findings.
Collaborating with counselors, case managers, or other treatment team members to coordinate client care.
Timely and accurate completion of medical records, including assessment, treatment plan, progress notes, outcome measures, and billing entries.
Examining or ordering laboratory or diagnostic tests for clients to provide information on general physical condition and mental disorders.
Ordering, obtaining, and interpreting appropriate diagnostic tests.
Designing, ordering, and documenting appropriate treatment plans/plans of care including prescriptions of medications based on a comprehensive review of health information and diagnostic results.
Initiating protocols/clinical pathways and evaluating the plan of care ensuring timely intervention.
Performing invasive procedures when needed.
Collaborating with primary care providers regarding admission, discharge, and transfer decisions.
Presenting patients in the caseload in rounds.
Communicating plan of care with appropriate providers including the collaborative physician, the primary care provider, the referring provider, the nursing staff, and the case manager.
Communicating plans of care with the patients and family members.
Providing patient and family instruction related to the plan of care, disease process, new treatment plans, and medication regimens.
Counseling and educating patients and family members in disease processes, health maintenance and health promotion activities.
Providing primary, secondary, and tertiary preventive care services.
Initiating appropriate referrals and specialty consultations or other agency involvement as needed.
Facilitating patient flow.
Collaborating with the multidisciplinary team including case management ensuring appropriate and timely discharge.
Providing specialty consultation.
Advocating for patients ensuring patient/family participation in care, knowledge of treatment options, and understanding of patient rights.
Considering the holistic needs of patients and orchestrating resources (i.e., pain service, palliative care).
Performing other duties as assigned.
Providing formal and informal educational programs for other members of the health care team consistent with evidence-based practice standards.
Participating in mentoring activities.
Precepting interns.
Promoting understanding of disease processes in area of specialty/program.
Functioning as a clinical resource for other team members including but not limited to interns, residents, fellows, nurses, and medical students.
Providing community education, health promotion, and outreach.
Acting as a community health resource.
Coordinating adequate nurse practitioner coverage on their service.
Working with the physician and nurse practitioner staff to ensure this coverage.
Working with primary care providers to achieve service specific outcome targets for key metrics including clinical outcomes, level of service, cost of care and patient satisfaction.
Participating in performance improvement activities aimed at improving clinical outcomes and minimizing variation.
Developing clinical pathways in collaboration with physician leadership and the entire leadership team.
Participating in and leading quality assurance activities (i.e., chart reviews, peer review) and establishing standard of practice.
Advancing the patient care delivery process through the application of research, evidence-based practice standards, and industry best practices.
Advising and influencing CCG policies and procedures to improve the delivery of care.
Introducing and evaluating new patient care delivery systems, models of care and therapeutic and preventive interventions that target patient needs not met by current care delivery strategies.
Actively participating and contributing to various department and division meetings and organizational initiatives.
Maintaining compliance and regulatory documentation for administrative purposes and reimbursement for services.
Participating in accreditation readiness.
Supervising ancillary staff including completing annual reviews.
Modeling and upholding a customer service focus to all internal and external customers.
