A minimum of one (1) year of independent outpatient clinical practice experience is required, and providers with three to five (3–5) years of independent primary care experience are strongly encouraged to apply. Greater Seacoast Health is seeking an experienced Nurse Practitioner (APRN) or Physician Associate (PA) to join our growing Primary Care team in Somersworth, New Hampshire. Requirements

  • Current New Hampshire APRN and RN licensure.
  • Board Certification through ANCC and/or AANP.
  • Current DEA registration.
  • Current BLS certification.
  • Current New Hampshire Physician Associate license.
  • NCCPA Certification.
  • Minimum of one (1) year of independent outpatient clinical practice experience.
  • Demonstrated experience independently managing an established primary care patient panel.
  • Experience providing comprehensive primary care services across the lifespan.
  • Experience diagnosing and managing acute and chronic medical conditions across the lifespan.
  • Strong commitment to team-based care, quality outcomes, and population health management.

Nice To Haves

  • Providers with three to five (3–5) years of independent primary care experience are strongly encouraged to apply.
  • Primary Care, Family Medicine, Internal Medicine, Community Health, Urgent Care, or FQHC experience is strongly preferred.
  • Bring established outpatient primary care experience.

Responsibilities

  • Deliver comprehensive primary care services across the lifespan.
  • Perform preventive care, wellness visits, and chronic disease management.
  • Diagnose and treat acute and chronic medical conditions.
  • Independently manage an established patient panel.
  • Coordinate care with specialists and community resources.
  • Collaborate closely with behavioral health, care management, and support teams.
  • Participate in quality improvement and population health initiatives.
  • Document patient care accurately within the electronic health record.
  • Help foster a culture of teamwork, compassion, and continuous improvement.
