Join an interdisciplinary Adult Mental Health and Substance Use (MHSU) team providing community-based care to adults experiencing moderate to severe mental health, substance use, and concurrent disorders. Working within a neighbourhood-based team model, you will collaborate with psychiatrists, addiction medicine physicians, nurses, social workers, counsellors, occupational therapists, peer support workers, and other allied health professionals to provide comprehensive, person-centred care. This Nurse Practitioner role will focus on substance use services, supporting individuals with complex and persistent substance use challenges who often experience significant barriers related to housing, poverty, stigma, physical health, and access to care. As part of an integrated team, you will provide assessment, treatment, care coordination, and longitudinal support for some of the region's most vulnerable populations while helping expand access to specialized substance use care across Vancouver Community Mental Health and Substance Use Services.
