About The Position

Catholic Charities of Long Island (CCLI) is seeking a part-time Nurse Practitioner for its Talbot House, a 24-hour residential stabilization center licensed by the New York State Office of Addiction. CCLI's mission is to provide dignified and compassionate care to vulnerable individuals, inspired by the Gospel call to affirm the sacredness of human life. The role involves contributing to the well-being of others and providing 'care with dignity and life with hope.'

Requirements

• Knowledge of physical health and substance use disorder treatment modalities.
• Trauma-informed and client-centered care approach.
• Master’s degree in Nursing and must be eligible for NYS Board Certification as Nurse Practitioner.
• At least 2 years’ relevant experience demonstrating strong clinical assessment and diagnostic skills.
• NY Nurse Practitioner License.

Nice To Haves

• Medication-assisted treatment (MAT) experience preferred.

Responsibilities

• Diagnose and treat patients in accordance with advanced nursing practices and established protocols, writing and/or participating in a plan of care for each individual.
• Order and evaluate appropriate medical regimens including medications and relevant tests.
• Identify clients in need of further evaluation or specialized care, making referrals as appropriate.
• Collaborate with other health disciplines to ensure care continuity and facilitate treatment progress.
• Provide health counseling to clients to expedite recovery, stability and long-term wellness.
• Consult with Medical Director/staff psychiatrist and professional staff when needed.
• Monitor and maintain appropriate records.
• Comply with governing regulations and Agency policies and procedures.
