About The Position

Catholic Charities of Long Island is seeking a part-time Nurse Practitioner for their Talbot House, a 24-hour residential stabilization center licensed by the New York State Office of Addiction. The role involves providing dignified and compassionate care to individuals with substance use disorders, contributing to the organization's mission of offering "care with dignity and life with hope."

Requirements

• Knowledge of physical health and substance use disorder treatment modalities.
• Master’s degree in Nursing and must be eligible for NYS Board Certification as Nurse Practitioner.
• At least 2 years’ relevant experience demonstrating strong clinical assessment and diagnostic skills.
• NY Nurse Practitioner License.

Nice To Haves

• Medication-assisted treatment (MAT) experience preferred.
• Trauma-informed and client-centered care approach.

Responsibilities

• Diagnose and treat patients in accordance with advanced nursing practices and established protocols, writing and/or participating in a plan of care for each individual.
• Order and evaluate appropriate medical regimens including medications and relevant tests.
• Identify clients in need of further evaluation or specialized care, making referrals as appropriate.
• Collaborate with other health disciplines to ensure care continuity and facilitate treatment progress.
• Provide health counseling to clients to expedite recovery, stability and long-term wellness.
• Consult with Medical Director/staff psychiatrist and professional staff when needed.
• Monitor and maintain appropriate records.
• Comply with governing regulations and Agency policies and procedures.
