About The Position

This role brings advanced clinical expertise beyond the hospital walls and into the home, allowing for a more holistic patient view. Practitioners will practice with greater autonomy, deliver high-acuity care in a personal setting, and use clinical judgment to prevent complications, reduce hospitalizations, and make a lasting difference. The role involves performing hands-on assessments, independent IV access, medication administration, wound care, and splinting autonomously. The Nurse Practitioner reports to the Field Provider Manager and has the freedom to assess, decide, and act.

Requirements

Current, unrestricted state licensure as an NP (ARNP) or PA-C in Washington.
U.S. residency and eligibility to work without sponsorship.
Minimum of 2 years (or 3,600 hours) of clinical experience specifically in acute settings: Emergency Department, Inpatient Medicine (hospitalist, med-surg, telemetry, step-down), or Acute Post-Acute (LTACH, acute rehab, SNF).
Comfortable operating as the solo on-site clinical authority.
Demonstrated procedural fluency in independent IV access, medication administration, and point-of-care diagnostics.
Active, unrestricted driver's license.
Physical capacity to carry up to 60 lbs of equipment, navigate stairs, and deliver unpredictable, autonomous care in non-clinical home environments.
Current BLS certification.
Flexibility for evenings, weekends, and holiday coverage.

Nice To Haves

AGACNP-BC, ENP-BC, or PA-C with NCCPA CAQ in Emergency or Hospital Medicine.
5+ years of acute practice.
Pre-licensure EMS background (paramedic/critical care transport).
Previous Hospital-at-Home experience.
ACLS certification.

Responsibilities

Perform problem-focused assessments and deliver therapeutic interventions (e.g., IV fluids, medication administration) in the home.
Order and interpret diagnostic tests (labs, EKGs, POCUS).
Collaborate with referring clinicians to create holistic care plans that integrate patient goals and palliative principles.
Participate in quality initiatives that have a measurable impact.
