About The Position

Santa Rosa Behavioral Healthcare Hospital (SRBHH) is seeking a Nurse Post Discharge Coordinator. This role is integral to the mental health system of care, serving adolescents and adults with mental health and co-occurring psychiatric and substance abuse conditions. The hospital is located in the North Bay and has 144 acute psychiatric inpatient beds. The Nurse Post Discharge Coordinator will collaborate with a multidisciplinary team to provide quality services in discharge planning, crisis calls, referring to community-based resources, data collection, and documenting patient medical status post-discharge. This role follows the Zero Suicide protocol and supports the hospital's nursing department in a hybrid capacity. The coordinator is a member of the interdisciplinary team, supporting the organization's treatment program and philosophy, and ensuring quality treatment delivery to patients and their support systems.

Requirements

  • Licensed as an RN, LVN, or LPT in the state of California that is active and current.
  • One year of behavioral health experience and experience with Severely Mentally Ill and discharge planning preferred.
  • Current CPR (Cardiopulmonary Resuscitation) certification (or obtained within 30 days of hire).
  • Current Handle with Care (HWC) certification (or obtained within the 30 days of hire).
  • Familiarity with continuum of care and clinical terminology.
  • Maintains confidentiality of patients at all times
  • Sensitivity to and willingness to interact with persons of various social, cultural, economic and educational backgrounds.
  • Proficiency with software and/or equipment (Microsoft Office applications including Outlook, Word, Excel and PowerPoint.)
  • Strong organizational skills with ability to prioritize projects, work relatively independently, manage multiple tasks, and meet deadlines.
  • Strong written and verbal communication skills.
  • Ability to work independently and as part of a team.

Responsibilities

  • Manages discharge reference tool/discharge calendar to ensure notification and documentation of daily discharges.
  • Manages post-discharge tracking tool to ensure quality assurance measures for performance improvement.
  • Manages and updates resource guides/lists per county and consumer as needed.
  • Facilitates post-discharge coordination of care, linkages to appropriate resources, and connections to aftercare services to reduce recidivism rates.
  • Ensures follow-up appointments are arranged/scheduled for psychiatrist and therapist within 7 days of discharge.
  • Facilitates C-SSRS when calling a former patient who reports suicidality. For high scores, ensures linkages to crisis services specific to their county/location.
  • Review and facilitate safety planning with former patients who require additional support. References Stanley Brown Safety Plan as needed.
  • Documents all services provided in the patient’s EMR.
  • Checks voicemails daily and provides a call back within 24 hours.
  • Maintains effective working and professional relationships and communications with internal and external customers.
  • Supports department in administrative duties, auditing, and tracking data for QAPI and compliance.
  • Assists with training and education to staff regarding measurable outcomes from data tracking, including updated resource information.
  • Assists with training other staff competency with elements of this role as determined by hospital leadership.
  • Upholds the Organization’s ethics and customer service standards.
  • Supports Nursing department with related tasks, and provide direct patient care when requested.
  • Performs other duties, as assigned. Tasks are subject to change based on the growing needs of the hospital.
