About The Position

The Nurse Navigator serves as a single point of contact for the referring provider, patients, and caregivers to provide resources and assistance with accessing clinical and supportive care services offered by EvergreenHealth and in the community. Facilitate patient appointments, including labs, diagnostic areas, and specialty physicians to ensure all patients receive quality and comprehensive services. Functions on the multidisciplinary team as an advocate, educator, and counselor for Women’s Services patients.

Requirements

  • Bachelor’s in Nursing (BSN) required within 5 years of employment if applicable
  • Licensed as a Registered Nurse in the State of Washington
  • 3 years of clinical perinatal or behavioral health experience
  • Minimum of one year ambulatory clinic experience
  • Current Healthcare Provider BLS – American Heart Association by start date

Nice To Haves

  • Master’s degree
  • 5 years of clinical experience in perinatal, behavioral health or care management

Responsibilities

  • Serves as an essential link between patients and all other care providers.
  • Assists patients in understanding their diagnosis, treatment options, and the resources available.
  • Works with patient and care team members to create an individualized plan of care.
  • Educates eligible patients about appropriate supportive care resources, research studies and technologies available to this unique patient population.
  • Follows patients throughout the care continuum, including inpatient admissions, and collaborates with inpatient, outpatient, and community care management resources.
  • Responsible for the outreach efforts to establish and maintain positive working relationships with key customers (physicians, office staff, nurses, social services staff, etc.).
  • Performs other duties as assigned.
