About The Position

The Nurse Navigator will support patients and their families during their course of treatment at the New York Proton Center, including, but not limited to, pre-treatment education, assessing higher-level-care needs, and communicating with patient care teams. Additional responsibilities for this role include collaborating with appropriate agencies to advocate for services, following up on the patient’s status, and providing clinical care, as needed. The Nurse Navigator will provide high quality customer service to our patients and their families.

Requirements

  • Registered nurse
  • Basic Life Support (BLS) certification
  • Ability to identify resources and utilize problem solving skills in order to meet patient’s individual needs
  • Strong written and verbal communication skills
  • Attention to detail and problem solving skills

Nice To Haves

  • Prior oncology experience, preferred
  • Minimum of 3 years medical office or clinical environment experience, preferred
  • Strong IV placement skills, preferred

Responsibilities

  • Listen to patient and caregiver questions and give well informed answers in a way patients and caregivers can understand, and know when to defer questions to appropriate care-team members
  • Educate and prepare patients and their families prior to their NYPC treatment planning appointment, including referring to additional resources, as needed.
  • Facilitate patient’s first NYPC appointment with treatment planning team including: obtaining consent(s), placing IVs, and administering contrast per established protocol.
  • Guide patients throughout their treatment course and help facilitate the planning, scheduling, and implementation of day-to-day clinical activities
  • Provide personalized support for higher-risk patients i.e., patients with limited support system or resources, etc.
  • Collaborate with patient care team to better understand the social and emotional factors potentially impacting the patient’s health status
  • Monitor patient adherence to their NYPC appointments and notify patient care team of compliance concerns
  • Provide critical feedback to other members of the NYPC clinical team by participating in multi-disciplinary team meetings
  • Coordinate chemotherapy appointments with proton therapy appointments, as needed
  • Arrange referrals and appointments for New York Proton Center patients with other medical providers, including surgeons, medical oncologists, medical subspecialists, gynecologists, dentists, etc., as needed
  • Utilize quantitative measures to monitor patient progress
  • Monitor patient satisfaction surveys / complaints
  • Comply with all New York Proton Center policies related to Privacy Rules established under the Health Information Privacy and Portability Act of 1996 (HIPAA)
  • Perform other job-related duties, as assigned
