About The Position

The Ambulatory Mary Parkes Asthma & Pulmonary Center Nurse Navigator provides individualized assistance, support, and comprehensive coordination of care for patients and their families/caregivers. The role aims to eliminate barriers to timely care, facilitate patient flow through the system, enhance interactions with the care team, improve patient and provider satisfaction, and serve as a consistent point of contact. This position is full-time, working day shifts from 8:30 AM to 5:00 PM, Monday through Friday, within the Airways Disease Program at the Mary Parkes Asthma & Pulmonary Center. The Nurse Navigator will collaborate closely with pulmonary specialists to deliver comprehensive care to patients with Asthma, COPD, and Bronchiectasis, guiding them through their care journey.

Requirements

3 years of experience
Ambulatory experience preferred

Responsibilities

Provides individualized assistance, support and comprehensive coordination of care for patients and their families/caregivers.
Eliminates barriers to timely care, facilitates flow through system, and facilitates interactions with care team.
Increases patient and provider satisfaction and maintains point of contact with providers.
Serves as first point of contact for patients and families.
Coordinates the entire care delivery process for Asthma, COPD, and Bronchiectasis patients, starting from their initial referral.
Arranges post-hospital visits, provides essential education, and promotes adherence to treatment plans.
Connects high-risk patients with home care resources.
Acts as a liaison for the prior authorization process for specialty medications and necessary equipment.
Develops and maintains a registry specifically for high-risk patients within the Center.
Attends bi-monthly Airways Disease conferences to stay current and contribute to program development.
