About The Position

Incumbent will provide nursing care to patients, coordination and oversight in the outpatient Urology clinic. Incumbent serves as a single point of contact for patients and/or family members for the purpose of effectively communicating the patient's plan of care and coordinates care with the physicians and members of the healthcare team.

Requirements

  • Strong written, oral communication, as well as effective problem solving and negotiation skills.
  • Must possess the skill, knowledge and ability essential to the successful performance of assigned duties.
  • Must demonstrate customer service skills appropriate to the job.
  • Minimum 1 year experience working as an RN or 3 years MA/LVN experience in a clinical setting.
  • Experience coordinating care for patients with complicated medical diagnosis.
  • Excellent written and verbal communication skills in English.
  • Current California RN license.
  • Current BLS certification.
  • Computer familiarity using Word, Outlook and Excel, or similar database software.
  • All external candidates must have a Bachelor of Science in Nursing.
  • Ability to maintain a work pace appropriate to the workload.
  • Ability to establish and maintain effective working relationships across the Health System.
  • Must be able to provide proof of work authorization.

Nice To Haves

  • Specialty nursing certification relating to patient population.
  • Previous experience working as a nurse navigator (care coordinator) for patients with complicated medical diagnosis, preferably urology.
  • Previous experience working as a nurse navigator (care coordinator) for patients with complicated medical diagnosis, preferably pulmonary and/or nephrology clinics.
  • Knowledge of University and medical center organizations, policies, procedures and forms.

Responsibilities
