About The Position
Incumbent will provide nursing care to patients, coordination and oversight in the outpatient Urology clinic. Incumbent serves as a single point of contact for patients and/or family members for the purpose of effectively communicating the patient's plan of care and coordinates care with the physicians and members of the healthcare team.
Requirements
- Strong written, oral communication, as well as effective problem solving and negotiation skills.
- Must possess the skill, knowledge and ability essential to the successful performance of assigned duties.
- Must demonstrate customer service skills appropriate to the job.
- Minimum 1 year experience working as an RN or 3 years MA/LVN experience in a clinical setting.
- Experience coordinating care for patients with complicated medical diagnosis.
- Excellent written and verbal communication skills in English.
- Current California RN license.
- Current BLS certification.
- Computer familiarity using Word, Outlook and Excel, or similar database software.
- All external candidates must have a Bachelor of Science in Nursing.
- Ability to maintain a work pace appropriate to the workload.
- Ability to establish and maintain effective working relationships across the Health System.
- Must be able to provide proof of work authorization.
Nice To Haves
- Specialty nursing certification relating to patient population.
- Previous experience working as a nurse navigator (care coordinator) for patients with complicated medical diagnosis, preferably urology.
- Previous experience working as a nurse navigator (care coordinator) for patients with complicated medical diagnosis, preferably pulmonary and/or nephrology clinics.
- Knowledge of University and medical center organizations, policies, procedures and forms.
Responsibilities
- Provide nursing care to patients, coordination and oversight in the outpatient Urology clinic.
- Serve as a single point of contact for patients and/or family members for the purpose of effectively communicating the patient's plan of care.
- Coordinate care with physicians and members of the healthcare team.
- Design and implement patient and family education programs.
