About The Position

The Nurse Navigator (RN) at Adena Health provides professional nursing care through the application of nursing skills and processes to develop, implement, and evaluate the patient's plan of care. This role creates an atmosphere that supports the vision of being the region's most trusted partner in healthcare. As the primary caregiver, the Nurse Navigator provides direct patient care and collaborates with other care team members, physicians, and patients/families to achieve optimal patient outcomes. The incumbent has access to confidential patient information and makes decisions independently and in collaboration with others. This position works under the direct supervision of the Nurse Manager and the indirect supervision of the Director of Nursing.
