About The Position

The Nurse Navigator acts as a liaison between patients, physicians, and the respective care team. This role is responsible for coordinating care across the continuum, providing education and support to patients, facilitating decision-making, and advocating for patients to improve throughput and satisfaction. The Nurse Navigator supports individuals and significant others throughout the continuum of care in all settings. Additionally, the Nurse Navigator monitors patient outcomes and participates in quality improvement activities. This position involves working in a multidisciplinary setting and collaborating with healthcare team members to positively impact patient outcomes and experiences.

Requirements

  • Bachelors degree
  • Neonatal Resuscitation Program (NRP) certification
  • Pediatric Advanced Life Support (PALS) certification
  • Basic Life Support (BLS) certification
  • Registered Nurse (RN) license
  • Advanced Cardiovascular Life Support (ACLS) certification
  • Bachelor Science Nursing (BSN)
  • Must have an active Registered Nurse license
  • Three years of clinical experience working in a hospital, managed care environment or medical office
  • Life Support Certificate (BLS, NRP, PALS, ACLS) as per department standard
  • For South Entities, RNs hired after 2/2012 with an Associates degree have 3 years to complete the BSN degree

Nice To Haves

  • For BHQN, knowledge of HEDIS and Star ratings, risk adjustment and HCC coding preferred
  • Proficient in computer applications and typing skills
  • Experience in Electronic Medical Record systems preferred
  • Bilingual preferred
  • Preferred certification in area of expertise
